Daniel’s Story

Daniel (name changed) was referred to the Community Wellbeing Pathfinder service following a recent diagnosis of Multiple Sclerosis (MS).

Living in one of Newcastle’s most deprived communities, he was experiencing poor mental wellbeing, financial difficulties and increasing social isolation, leaving him unsure where to turn for support.

What Healthworks did:

Over four one-to-one sessions, Daniel worked with a Community Wellbeing Pathfinder to identify his health and wellbeing priorities and build confidence.

He received personalised support, was connected with mental health and financial advice services, and was introduced to local MS support groups to help reduce isolation and improve his wellbeing.

Community Wellbeing Pathfinder Outcomes:

Daniel developed a stronger support network, improved his mental wellbeing and gained the confidence to better manage his condition.

His wellbeing score improved, alongside positive progress across multiple Outcome Star domains.

The support I received has been invaluable.

It gave me the confidence to access the right services, connect with others living with MS and feel much more positive about managing my condition and my future.
