Sarah’s Story

Following a diagnosis of early-onset Alzheimer’s, Sarah (name changed) wanted support to maintain her independence and improve her health and wellbeing.

She was keen to stay active, build her confidence and continue enjoying everyday life.

What Healthworks did:

Sarah received personalised one-to-one support to help her identify and work towards her health and wellbeing goals.

With encouragement and guidance, she increased her physical activity, made healthier lifestyle choices and started reconnecting with friends.

Community Wellbeing Pathfinder Outcomes:

Sarah lost 7kg, became more physically active and gained confidence managing her daily life.

She also reconnected with friends and is now working towards an exciting personal goal of taking part in the Great North Run.

“Sessions felt easy and comfortable with no judgement. I looked forward to our meetings.”
