At Healthworks, we know that looking after your health and wellbeing isn’t always easy—and sometimes, a bit of extra support can make all the difference.
That’s why we’re excited to introduce our Community Wellbeing Pathfinder Service, designed to support you in taking the next step toward a healthier, happier life.
What is the Community Wellbeing Pathfinder Service?
Our Pathfinder team offers one-to-one support to help you improve your overall wellbeing and navigate life’s challenges. Whether you’re looking to improve your lifestyle, find work, manage your health, or feel more connected in your community, we’re here for you.
We work with adults 18+ who live in Newcastle, offering personalised guidance in the way that suits you best—face-to-face, over the phone, or by video.
How We Can Help
You can access support in areas such as:
- Achieving a healthier lifestyle
- Managing physical health and learning self-care techniques
- Finding jobs or volunteering opportunities
- Improving housing and financial wellbeing
- Reducing loneliness and building social support
- Mental health and emotional wellbeing support
We’ll provide:
- Personal signposting
- Trusted advice and information
- Referrals to local services
- Tailored action plans to help you move forward with confidence

We are accepting referrals ahead of the service launch. To refer yourself to the Community Wellbeing Pathfinder Services, complete the online referral form.
