Join the PERFORM Trial at Healthworks Newcastle
Personalised Exercise Rehabilitation for Long-Term Health Conditions
We are excited to announce the launch of the PERFORM Trial at Healthworks Newcastle, a groundbreaking research project aimed at supporting individuals living with multiple long-term health conditions.
If you have two or more long-term health conditions such as diabetes, asthma, arthritis, Long COVID, hypertension, chronic pain, cancer, or heart and lung conditions, you may be eligible to take part in this exercise-based rehabilitation trial.
What’s Involved?
Participation in the study includes:
- Baseline Assessment Visit
- 8-week rehabilitation programme
- Follow-up visits at 3 months and 12 months
The PERFORM Exercise-Based Rehabilitation Programme includes:
- Initial physical assessment
- Group sessions twice a week for 8 weeks
- 1st hour: Group exercise (warm-up, strength and aerobic training, cool-down)
- 2nd hour: Health and wellbeing presentation
- Post-programme support including an individual discharge appointment and group check-ins at 2 and 4 months after your last session.
In collaboration with the National Institute for Health Research (NIHR), the University of Leicester, and Northumbria University, the PERFORM Trial is developing a personalised rehabilitation programme tailored to individual needs. The goal is to help participants feel better, stay active, and reduce hospital visits through safe and effective exercise.
